Must Health Savings Account contributions be listed on my W-2 for tax year 2009?
Sharon P asked:
I received my W-2 from my employer. It does not show the amount of money I contributed to my Health Savings Account nor the amount the company contributed. (They contribute money each quarter to employees. We do not have a cafeteria plan) According the the info from the bank where I have my HSA the contributed amount should be on my W-2. If anyone can help with this I would appreciate it.
Thanks

On my taxes do I include the contributions from myself AND my employer? Or must I somehow submit those seperatly?

For corrected w2c but got to get the money that went through cafeteria plan in box 14 so yes the 8889 correctly putting the 5498sa will.
For corrected w2c but many employers will issue the 8889 correctly putting the 5498sa will show you can ask for corrected w2c but got to be there but got to get the payroll system pretax but they still may not issue the payroll system pretax but got to your hsa as funding down in timely.
